This is a Registration Request Form only. Submission does not guarantee enrollment. Registration is confirmed only after Gateway Ice Centre reviews your request, confirms availability, and issues an invoice.

Please do not submit payment prior to receiving your invoice. We must first review your registration request and confirm that a space is available for enrollment. Payment instructions will be provided with your invoice once your child’s spot has been confirmed.
Please Note: Registration requests are reviewed and processed during our regular reception office hours. We appreciate your patience while your request is being reviewed.


Registering More Than One Child?
A separate registration request form must be completed for each child.
If you are registering more than one child, please let us know below so we can consider availability for all children when reviewing your requests.
